Metro 24/7 FitnessTM Membership Cancellation Request

Please complete the entire form and only one form per cancellation. Once you have completed the form you can mail, fax, or bring into Metro 24/7 FitnessTM. If you fax or mail in the form, you must call to ensure that it was received. Keep in mind your call will be documented. As per your agreement, there is a $25 fee drafted from your account upon receiving cancellation notice to the automatic renewal program and you are required to give 30-days notice in advance of the scheduled billing date. You will be billed one more time after your cancellation request has been submitted. This will start from the date your cancellation is received by Metro 24/7 FitnessTM.
